Deposit:

All appointments require a $75 NON-REFUNDABLE deposit at the time of booking. The deposit will be applied to the cost of your treatment. 

Rescheduling:

You are able to reschedule your appointment once. After one reschedule, your deposit becomes forfeit and a new one will be required to keep your appointment.

Late Cancellation:

If you cancel/reschedule your appointment within 48 hours you will be charged 50% of your scheduled service (and $200 for Botox appointments), no exceptions.

No-Show:

If you no-show your appointment you will be charged 100% of your scheduled service (and $400 for Botox appointments), no exceptions.

We reserve the right to refuse service for failure to abide by our booking policy or failure to pay our late cancellation/no-show fees. If you fail to pay your late cancellation/no-show fees, you will be personally billed for those charges. By booking your appointment you agree to the above policies and authorize us to charge your credit card on file for applicable fees.

RECORD REQUESTS

All records request will incur a $10 flat fee plus $0.25 cents per page administrative fee.

Please allow 30 days from when we receive your release form for records to be available. 

GIFT CERTIFICATE TERMS & CONDITIONS 

USE

Usable up to balance only at Rejuvn8, LLC (“Rejuvn8”). Valid only for services rendered and products sold by Rejuvn8 including: skincare, skincare products, medical aesthetics and wellness services, not for cash refunds. If the gift certificate holder’s total purchase exceeds the value of the gift certificate, the holder is required to pay the difference to complete the purchase. 

EXPIRATION

Any gift card rendered by Rejuvn8 shall expire five years from the date of issue or the date on which funds were last loaded.

SERVICE FEE

After twenty-four consecutive months of non-use, a monthly service fee shall be imposed against the balance of the gift certificate. The monthly service fee will be equal to 3% of the original value of the gift certificate. This fee shall be waived, and the gift certificate replenished to its value prior to the fee being assessed when the holder of such gift certificate presents the certificate within three years of the date of its issuance. 

RISK OF LOSS

Gift certificates will not be replaced if lost, stolen or used without authorization. If required by law, a fee of $5 shall be imposed for replacement of a gift certificate. 

LIMITATIONS

All sales of gift certificates are final and non-refundable. Gift certificates will not be redeemable for cash under any circumstances. 

GENERAL TERMS

Rejuvn8 is the certificate issuer and sole obligor to the certificate owner. Rejuvn8 may freely delegate its issuer obligations to an assignee without recourse. If delegated, the assignee and not Rejuvn8 will be the sole obligor to the certificate owner. The terms and conditions applicable to the purchase and redemption of Rejuvn8 gift certificates are governed by the laws of the State of New York. All terms and conditions are applicable to the extent permitted by law.
